I had a nose job a couple years ago from a doctor in SoCal who is now retired. I am satisfied with the shape. However the nostrils do not seem symmetrical. Should I have this corrected or just forget about it?
Symmetry is a challenge in facial plastic surgery and most facial plastic surgeons discourage patients from having symmetry as a goal of surgery. Most patients have underlying facial Asymmetries and it is important to appreciate these asymmetries for what they are and that while we may be able to achieve LESS Asymmetry, the goal of symmetry should be discouraged.

I’m looking to have a chin augmentation, but does the implant feel fake? Would my husband think it felt unnatural?
During the initial healing process, you will have some swelling and you may have some numbness of the area of the chin/lower lip; these sensations may be perceived as unnatural. However, after the swelling has resolved and you have healed, the implant is natural in its look and feel.

Can malar bags be improved with an eyelid lift?
Malar bags are a challenge to treat. I would recommend an eyelid lift to address excess skin and/or fat of the eyelids, but I wouldn’t recommend an eyelid lift specifically for the treatment of malar bags. Malar bags have several potential causes, including but not limited to allergies, lymphatic drainage, salt intake. Several treatment options exist, with variable results, including but not limited to laser, surgical excision, eyelid lift, radiofrequency, injection of medication, such as steroid and/or hyaluronidase.

The area under my eyes is very hollow. Is it possible to have a fat transfer in this area?
Yes, it is possible to have fat transfer to hollow areas under the eyes. Other alternatives include injection of hyaluronic acid filler (Restylane, for example) and lower eyelid surgery (blepharoplasty) with fat transposition. The fat transposition procedure involves re-positioning your own fat from the lower eyelids into the areas of hollowness, thus providing a more youthful contour between the lower eyelids and cheeks.

I am considering chin augmentation. What are the chances that a chin implant can shift positions?
The risk of a chin implant shifting position is low. There are several maneuvers utilized in the procedure to minimize this risk. One technique I use is to limit the area of surgery so that the implant fits in a pocket underneath the tissues and against the chin bone; a smaller area of surgery minimizes the risk of movement of the implant. Also, I stitch the implant into position, thus further minimizing the risk of the implant shifting positions.

Some non-surgical options include filler augmentation. We use fillers to the cheeks to address volume loss. This provides a more youthful contour to the cheek. As well, we perform skin tightening procedures with laser-based or radiofrequency-based technologies. Finally, skin resurfacing with a laser or chemical peel can improve skin texture as well as address fine lines/wrinkles.

After my rhinoplasty, how long do I need to wear that metal cover? Will I be able to drive home after the surgery? I am planning on having local anesthesia.
The bandage along the bridge of the nose remains in place for one week; I remove it one week after surgery. I do not recommend you drive yourself to and from the surgery center. You will need someone to drive you. Although some rhinoplasty procedures may be performed under local anesthesia, the great majority of rhinoplasty surgeries I perform are with general anesthesia.
