Procedure Detail: This 53 year old male presented with an interest in improving not only the function of his nose but also the shape of the nose. Specifically, he desired removal of a dorsal hump (bump on the top of the nose. Dr. Batniji performed the following:
Laser turbinectomy/turbinoplasty to reduce the size of hypertrophic (enlarged) inferior turbinates, thus improving his ability to breathe through the nose.
Septoplasty to straighten the nasal septum, thus improving his ability to breathe through the nose.
Rhinoplasty via an external approach.
Bilateral spreader grafts to maintain the integrity of the middle nasal vault and internal nasal valve.
Dorsal hump reduction (profileplasty) with a rasp was then used for subtle refinement of the bony nasal dorsum. Cartilaginous dorsal hump was removed conservatively. The upper lateral cartilages were preserved to further maintain the integrity of the middle nasal vault and internal nasal valve.
Caudal extension graft to increase tip projection, tip rotation, and tip support.
Tip graft with morselized cartilage to provide subtle refinement to the nasal tip.

Procedure Detail: This 18 year old female presented to Dr. Batniji in consultation for a rhinoplasty. She was interested in refinement of the nasal tip and removal of a bump along the bridge of the nose. As well, the patient, who is of Hispanic ethnicity, wanted a result that would be appropriate for her ethnicity and complement her facial features. Dr. Batniji performed the following:
Septoplasty via a left sided hemitransfixion incision to correct the deviated septum and harvest septal cartilage for use in rhinoplasty.
Rhinoplasty via an external approach.
Dorsal hump reduction (profileplasty) with a rasp for subtle refinement of the bony nasal dorsum. Cartilaginous dorsal hump was removed conservatively. The upper lateral cartilages were preserved to further maintain the integrity of the middle nasal vault and internal nasal valve.
The internal nasal valves were reconstructed to not only maintain the nasal airway, but also to improve upon its function. An auto-spreader graft was formed on the right side. Since the left middle nasal vault demonstrated a convexity, a thick spreader graft was placed on the left side to address this convexity, thus providing a more harmonious shape to the middle nasal vault.
The lateral crura of the lower lateral cartilages were not in proper position, broad in width, and convex in shape, thus giving the bulbous appearance to the nasal tip. Therefore, Dr. Batniji performed cephalic margin trim to address the broad width of the cartilages. Then, Dr. Batniji re-positioned the lateral crura and placed bilateral lateral crural strut grafts (fashioned from the previously harvested septal cartilage) to provide a more natural shape to the tip. Interdomal, transdomal and tip position sutures were also placed to further refine the nasal tip shape.
Of note, no osteotomies were performed on this patient.
