Case #15199 - Rhinoplasty

Procedure: Rhinoplasty

Procedure Detail: This 20 year old female presented to Dr. Batniji in consultation for a rhinoplasty. She was interested in addressing the crooked appearance of the nose on frontal view and removal of a bump along the bridge of the nose.

Septoplasty via a left sided hemitransfixion incision to correct the deviated septum and harvest septal cartilage for use in rhinoplasty.
Rhinoplasty via an ENDONASAL/CLOSED approach.
Dorsal hump reduction (profileplasty) with a rasp for subtle refinement of the bony nasal dorsum. Cartilaginous dorsal hump was removed. The upper lateral cartilages were preserved to further maintain the integrity of the middle nasal vault and internal nasal valve.
The internal nasal valves were reconstructed to not only maintain the nasal airway, but also to improve upon its function. Spreader grafts were placed on both sides.
In an effort to achieve tip rotation, a columellar strut was placed.
Lateral and medial osteotomies to further straighten the nose.

Procedure Detail: This 16 year old female presented to Dr. Batniji in consultation for a rhinoplasty. She was interested in refinement of the nasal tip and removal of a bump along the bridge of the nose:

Septoplasty via a left sided hemitransfixion incision to correct the deviated septum and harvest septal cartilage for use in rhinoplasty.
Rhinoplasty via an external approach.
Dorsal hump reduction (profileplasty) with a rasp for refinement of the bony nasal dorsum. Cartilaginous dorsal hump was removed. The upper lateral cartilages were preserved to further maintain the integrity of the middle nasal vault and internal nasal valve.
The internal nasal valves were reconstructed to not only maintain the nasal airway, but also to improve upon its function. Spreader grafts were placed on both sides. Since the left middle nasal vault demonstrated a concavity, a thicker spreader graft was placed on the left side to address this concavity, thus providing a more harmonious shape to the middle nasal vault.
In an effort to achieve tip refinement, Dr. Batniji performed cephalic margin trim to address the broad width of the cartilages. Then, Dr. Batniji performed interdomal, transdomal and tip position sutures to further refine the nasal tip shape. Finally, Dr. Batniji placed a tip graft that was obtained from the previously harvested septal cartilage; he crushed/softened the edges of the graft so that it appears natural without sharp edes.
Lateral osteotomies to close an open roof deformity.

Procedure Detail: This 16 year old male presented to Dr. Batniji in consultation for evaluation of severe nasal deformity and near-total nasal obstruction.  Dr. Batniji noted a severe septal deviation resulting in a crooked appearance to the nose on frontal view; indeed, the nasal tip falls off to the right side.  As well, the patient, who is of Hispanic ethnicity, wanted a result that would be appropriate for his ethnicity and complement his facial features.  Dr. Batniji performed the following:

Septoplasty via an external approach; Dr. Batniji chose an external approach to the septum because the caudal septum was severely deviated.  During the septoplasty, Dr. Batniji harvested septal cartilage for future use in the rhinoplasty.
Rhinoplasty via an external approach.
Dorsal hump reduction (profileplasty) with a rasp for subtle refinement of the bony nasal dorsum.  Cartilaginous dorsal hump was removed conservatively.  The upper lateral cartilages were preserved to further maintain the integrity of the middle nasal vault and internal nasal valve.
Caudal extension graft to reconstruct the severely deviated native caudal septum and provide nasal tip support as well as rotation and projection.
The internal nasal valves were reconstructed to not only maintain the nasal airway, but also to improve upon its function.  Bilateral spreader grafts were fashioned from previously harvested septal cartilage to reconstruct the internal nasal valves.
Lateral osteotomy of the left nasal bone to provide a more symmetric nasal bony pyramid.
Interdomal and transdomal sutures were placed to refine the nasal tip shape.  An infratip lobule graft and horizontally-oriented tip graft were placed to further refine the nasal tip.  Crushed cartilage was placed along the tip grafts to provide camouflaging of the tip grafts.

Procedure Detail: This 23 year old male presented with an interest in improving not only the function of his nose but also the shape of the nose. Specifically, he desired removal of a dorsal hump (bump on the top of the nose). Dr. Batniji performed the following:

Septoplasty to straighten the nasal septum, thus improving his ability to breathe through the nose.
Rhinoplasty via an external approach.
Bilateral spreader grafts to maintain the integrity of the middle nasal vault and internal nasal valve.
Dorsal hump reduction (profileplasty) with a rasp was then used for subtle refinement of the bony nasal dorsum. Cartilaginous dorsal hump was removed conservatively. The upper lateral cartilages were preserved to further maintain the integrity of the middle nasal vault and internal nasal valve.
Medial and lateral osteotomies.
Columellar strut.
Tip sutures to increase tip projection, tip rotation, and provide tip refinement.
Alar contour rim grafts to support the alar margins.
