I’m Asian and want to know how you would suggest changing a bulbous nasal tip and flared nostrils?
Flared nostrils can be addressed with nostril narrowing procedures. Through these procedures, I am able to remove the excess skin of the nostril and hide the incision in the crease between the nostril and the face. To address the bulbous nasal tip, I use suturing techniques to refine the appearance of the native tip cartilages. If the nasal skin is very thick, a tip graft is placed; the tip graft is fashioned from your own cartilage.

Is it true that augmentation is common in African American procedures and what does this entail?
Augmentaion of the African American dorsum or bridge of the nose is not uncommon. The method I choose for augmentation includes using cartilage (from either the septum, ear, or rib) diced and wrapped with temporalis fascia. Temporalis fascia is obtained from the scalp through a very small incision located above the ear.

HomeArticles Facial plastic and reconstructive surgeons Dr. Rami Batniji, lauded for his natural results (rhinoplasty leads the list) and remarkable patient care, discusses the evolution of eyelid procedures.
UPPER LIDS I am writing a paper for publication that describes my experience with a unique, minimally invasive endoscopic brow lift. I perform this surgery to not only reposition the brows, but also treat hooding of the upper eyelids.
LOWER LIDS Lower eyelid surgery has evolved. Rather than removing the fat from the lower lid, I reposition that fat into the hollowness between the lower eyelid and cheek. I then lift the cheek. This […]

Introduction
The key to successful nasal tip surgery is precise preoperative analysis, a thoughtful operative plan, and meticulous execution of that plan. To that end, an understanding of the anatomy of the nose, the pathology present, the desires of the patient and the maneuvers necessary to achieve […]
HomeArticles ABSTRACT
Many techniques have been developed for rejuvenation of the midface. This article describes the technique of subperiosteal midface elevation via an endoscopic brow incision approach developed by the senior author and reviews the results of this technique, with specific emphasis upon the effect on lower facial rejuvenation.
KEYWORDS: Endoscopicmidface lift, midfacial rejuvenation, lower facial rejuvenation, jowl, jawline
One of the goals of surgery for the aging face is to achieve the youthful contour of the neck and jawline. Techniques aimed at lower facial rejuvenation have demonstrated success in achieving this goal. Hamra’s technique of the composite rhytidectomy not only addressed the […]

How do you improve a flat, wide nasal appearance as often seen in African American noses?
The flat wide nasal appearance can sometimes be addressed by bringing the nasal bones inward via osteotomies. However, in African American noses, the nasal bones may be very short and wide and the bridge or dorsum of the nose may be flat. In this instance, augmentation of the dorsum will address the flat appearance and, on frontal view, provide the appearance of a narrower nose.

HomeArticles What does “pillow face” mean and how is it prevented?
Over time, we lose volume to the face, resulting in older, hollow appearance. One solution is the injection of filler material. If too much filler material is injected, the result can be large, unnatural lips and unnaturally large cheeks. This appearance of “over-injection” of filler material is known as “pillow face”. We avoid this appearance by adding volume to volume-deficient areas in a subtle manner, so that you may appear more youthful without looking overdone. Also, we tend to favor the use of hyaluronic acid fillers which are super forgiving […]
HomeArticles Is there a gel treatment for crow’s feet?
The treatment options I recommend for crow’s feet include Botox which softens the muscles that form crow’s feet. I also recommend a good eye serum, such as IS Clinical’s eye serum, to hydrate the skin around the eyes. If the crow’s feet are more prominent, I may recommend a laser resurfacing with fractionated CO2 laser or chemical peel with 35 percent TCA or 88 percent phenol. Interestingly, there are trials going on now for a botulinum toxin gel. The initial trials show some promise using this product for the treatment of crow’s […]
HomeArticles I am Asian-American and am seeking a consultation on rhinoplasty. I would like to have the bridge of my nose raised and narrowed and also have the width of my nose at the nostrils narrowed. I believe that this means some type of augmentation. Can this be done without detracting from my Asian features?
Yes, rhinoplasty can be performed without detracting from your Asian features. To narrow the nostrils, I look at the nostril width and the skin at the floor of the nose. Through a small incision placed strategically at the crease between the nostril and the face, the […]

Are there limits to what can be achieved with ethnic rhinoplasty?
Ethnic rhinoplasty limitations include the inherent strength and resilience of the nasal cartilages, the thickness of the nasal skin, and the healing process of the individual. Another consideration is the harmonious balance between the nose and the facial features. In ethnic rhinoplasty, we like to have as a goal a result that “fits the face” and is in harmony with other facial features, such as cheek bones, lips, mouth, eyes, and chin.
