Question:

I've had two photofacials, the first time I had positive results with my melasma and I was happy with it.  However, in the second photofacial my melasma got worst.  At this point I don't want to proceed with any more laser treatments, should I proceed with a  VI peel instead.  Will Vi peel help me with my melasma problem. What do you suggest?

Answer:

As you may know, melasma is a challenging skin condition to treat. In my experience, intense pulse light (IPL) treatments (also known as photofacials) are usually ineffective in the treatment of melasma and, in some instances, may worsen the appearance of melasma. We have a specific protocol for patients with melasma who undergo the VI Peel. This protocol incorporates a bleaching cream with 4% hydroquinone as well as a retinol. Furthermore, sunblock is an essential component for the management of melasma. Pre-treatment of the skin with a hydroquinone and retinol prior to the VI Peel may provide some added value in the ultimate result of the peel. Patients with melasma may benefit from a series (approximately 3) of VI Peel treatments.
